Primary Registered Nurse Responsibilities

The Primary Registered Nurse is responsible for being the primary case manager for patients who live in the community and within facilities. The primary nurse collaborates with float nurses to provide safe, consistent, and compassionate care to both the patient and their family. Flexibility, teamwork, and exceptional communication are a must for the primary nurse. The primary nurse collaborates with the interdisciplinary team to provide updates and to achieve high‑quality patient standards.

During periods when your caseload is lighter, you will be expected to support your team by assisting with patient visits and transitions, serving as a Float Registered Nurse as needed. This collaborative approach ensures that all patients receive the highest level of care, regardless of fluctuating caseloads. This role is in the Marinette territory and may travel outside of their territory at times.

Schedule

Full‑time, 36 hours per week (7:00 AM–4:00 PM, four days per week). Includes working a minimum of every 3rd weekend, although currently scheduled every 8th weekend. Also includes 10–15 rotating call‑back‑up shifts per year and approximately 1–2 holidays per year.

Key

Job Duties
  • Provide clinical and supporting case management based on patient’s diagnosis and end‑of‑life symptoms.
  • Complete initial, comprehensive, and routine assessments of patient and family needs.
  • Respond to urgent patient/family needs in collaboration with triage nursing staff.
  • Initiate crisis care as needed.
  • Develop, implement and review care plans for patient and family. Update care plan as the needs of the patient and family change.
  • Report appropriate findings and significant changes to the primary team, attending physician and primary caregiver.
  • Communicate with all interdisciplinary team members.
  • Teach family members patient care skills and encourage their participation including, but not limited to, pain control, medication management, dressings, hygiene and nutrition.
  • Provide education to patient/family members on disease progression, symptom management, medication and other associated patient care needs.
  • Administer and titrate medication according to physician/nurse practitioner orders.
  • Record all received orders to the Electronic Medical Record and initiate follow up as needed.
  • Delegate nursing duties to LPN and supervise care provided by CNA.
  • Prioritize daily work and make adjustments taking into account the needs of patients, Unity and other staff.
  • Complete new patient admissions as needed.
