Occupational Therapist, Home Health

We are seeking an Occupational Therapist, Home Health for Center Well Home Health
.

Competitive compensation: $77,200 - $106,200 per year plus $49.00 - $69.00 per visit/unit.

• Assess/screen patients’ daily living/work-related skills and develop therapeutic retraining programs with measurable objectives; administer and interpret diagnostic and prognostic tests of function in outpatient settings to assist physician evaluation.
• Confer with physicians and other health‑care team members; participate in development/revision of the Plan of Care Treatment.
• Provide therapeutic treatment and instruction in accordance with physician orders to improve/restore strength, coordination, range‑of‑motion and function or teach compensation measures.
• Review and expand retraining programs to align with behavioral goals and rehabilitation program guidelines.
• Consult with vocational team members to develop and implement vocationally oriented plans consistent with patients’ needs and capabilities.
• Report patient status and progress to physician, rehabilitation staff, Clinical Manager, patient and family members.
• Participate in care coordination and discharge planning activities and serve as a resource to other health‑care personnel.
• Design community reintegration activities to assist patients in physical reconditioning and psychological adjustment; coordinate plans with interdisciplinary team.
• Recommend and/or design special adaptive equipment to enhance residence or work environments and facilitate participation in the rehabilitation program and community.
• Design, train and supervise staff and family members in retraining program execution, including dressing, feeding, grooming and hygiene skills; participate in team and family educational meetings.
• Coordinate, monitor, supervise and evaluate Occupational Therapy Assistant and Home Health Aide performance in service delivery.
• Accurately document care observations, interventions and evaluations; submit treatment plans and discharge summaries to supervisor and care management staff.

• Degree from an accredited Occupational Therapy Program.
• Minimum of six months of occupational therapy experience (home health preferred).
• Current and unrestricted OT licensure.
• Current CPR certification.
• Good organizational and communication skills.
• Valid driver’s license, auto insurance and reliable transportation.
