This is a full‑time, Home Health Physical Therapist role based in Marion, OH. The Physical Therapist will evaluate and create personalized treatment plans to provide optimal patient care. Responsibilities include delivering therapeutic interventions, assessing patients' progress, collaborating with interdisciplinary teams, and ensuring documentation compliance. The role may involve coordinating rehabilitative services across various healthcare settings to help patients achieve their goals and improve their quality of life.
Qualifications- Proficiency in Physical Therapy and Rehabilitation techniques
- Hands‑on experience in Medicine, Patient Care, and Home Care settings
- Strong communication, organizational, and problem‑solving skills
- Ability to work collaboratively within interdisciplinary teams
- Current licensure as a Physical Therapist in the state of Ohio
- CPR certification and familiarity with applicable compliance standards
- Experience with electronic medical records is a plus
