Become the backbone of customer care as an Intake Hub Specialist with Para Med Home Health Care. This pivotal full-time role focuses on streamlining client referrals for improved service accessibility and coordination.
In this newly created position, the Intake Hub Specialist will manage end-to-end referrals for clients in need. Your responsibilities include conducting screenings, providing details on home care services, and documenting client information. The role demands strong operational judgment and proactive communication to ensure no referral is left unresolved.
Key Responsibilities:
• Manage inbound calls and online inquiries from clients
• Conduct initial screenings to determine service needs
• Provide information on programs and onboarding steps
• Update referral information in internal systems
• Track referrals to ensure timely service initiation
Requirements:
• 3+ years in home care or healthcare intake
• Strong communication and organizational skills
• Experience with customer-facing inquiries in fast-paced settings
• Proficient in Microsoft Office and CRM tools
• Familiarity with scheduling software like Alayacare
