Join Para Med Home Health Care as an Intake Hub Specialist dedicated to enhancing client experiences. This full-time role emphasizes compassion and efficiency in managing referrals while ensuring seamless care initiation.
In this unique position, you will handle various client inquiries and conduct screening to assess their specific service needs. Effective communication and strong judgement will be crucial as you navigate challenges and resolve service coordination issues. You'll also maintain accurate records to support timely referral processing.
Key Responsibilities:
• Receive and address inquiries from clients and referral partners
• Conduct assessments of client needs and urgency
• Share clear information about service offerings
• Maintain accurate client and referral data
• Monitor referral status for prompt service follow-up
Requirements:
• Minimum 3 years of relevant operational experience
• Strong client service and organizational skills
• Ability to manage high volumes of inquiries
• Experience with CRM and intake systems
• Knowledge of Alayacare is an asset
