Extendicare seeks a Registered Practical Nurse who will report to the Director of Care (DOC) or Director of Health and Wellness (DHW). You will deliver patient-centered nursing care, manage clinical assessments, and adapt care plans as needed. The role emphasizes teamwork with interdisciplinary staff to ensure quality care delivery for residents.
Key Responsibilities:
• Deliver planned, patient-oriented nursing care
• Administer medications and document care accurately
• Coordinate resident care assignments effectively
• Facilitate family education and care conferences
• Respond to significant changes in residents' conditions
Requirements:
• Current registration as a Registered Practical Nurse
• Experience with gerontology and unregulated care providers
• Strong communication and critical thinking skills
• Basic Cardiac Life Support and First Aid certified
• Computer proficiency in Microsoft Office applications
