Yee Hong Centre for Geriatric Care is a recognized leader in senior care, dedicated to providing exceptional, culturally inclusive services that enhance the well‑being of older adults. The organization serves more than 15,000 individuals annually across the Greater Toronto Area and operates 805 long‑term care beds along with additional facilities.
Responsibilities- Perform the functions of a Registered Practical Nurse in compliance with the College of Nurses of Ontario standards.
- Consult the Team Lead as appropriate about changes in residents’ condition, adverse events, or other necessary matters.
- Implement the nursing process: assessment, planning, implementation, evaluation, and documentation.
- Adhere to legislation, standards, policies, and procedures of the long‑term care home.
- Maintain regular and effective communication with family members for timely updates and support.
- Collaborate with attending physicians, laboratories, agencies, and hospitals to ensure appropriate delivery of services to residents.
- Initiate contact with appropriate team members when required by the resident’s condition to improve functioning.
- Plan and direct the work of Personal Support Workers assigned to the unit.
- Assist with new staff orientation and in‑service education.
- Provide ongoing feedback, coaching, and performance monitoring for Personal Support Workers.
- Report performance issues, incidents, and problems to the Assistant/Director of Resident Care.
- Participate in nursing‑related committees and coordinate interdisciplinary care conferences.
- Act as Unit Nurse during shifts and supervise student activities as required.
- Stay current with developments and best practices in the field.
- Perform other duties as assigned.
- Registered Practical Nurse with current Certificate of Competence from the College of Nurses of Ontario.
- CPR Certificate.
- Good leadership, initiative, problem‑solving, and judgment skills.
- Excellent customer service skills.
- Computer literacy.
- Satisfactory Police Vulnerable Sector Screen.
- Ability to converse in Chinese is an asset.
- Experience in nursing is an asset.
This position is temporary part‑time for 6 months, with the possibility of extension. The role requires considerable standing, walking, lifting, and bending. Exposure to all resident care elements through regular resident contact is expected. Shift work includes days, evenings, nights, and weekends.
Salary$28.95 – $32.06 per hour.
