S.R.T. Med Staff needs experienced Registered Practical Nurses for its Community Visiting Nursing Program. In this role, you will provide safe nursing care at various locations while collaborating with healthcare teams. Your expertise will help formulate effective care plans and ensure patient comfort at home, working various shifts to meet needs.

Key Responsibilities:

• Formulate care plans in collaboration with patients and families

• Provide nursing care based on the written care plan

• Evaluate treatment efficacy and adjust as needed

• Advocate for patient rights and safety

• Share nursing expertise with healthcare providers

Requirements:

• Registered Practical Nurse with the College of Nurses of Ontario

• One year of recent community experience

• Excellent problem-solving and communication skills

• Valid CPR certification and driver’s license

• Ability to provide a recent VSS
