In this position, you will be an integral part of the care team focused on delivering high-quality stroke rehabilitation services. You'll coordinate patient care, ensuring continuity and education throughout their stay. This role also requires you to utilize your assessment skills and collaborate with fellow professionals to enhance patient care plans.
Key Responsibilities:
• Assess patients comprehensively for tailored care
• Coordinate development of interprofessional care plans
• Educate patients and families during treatment
• Implement evidence-based nursing practices
• Work with the team on patient care program improvements
Requirements:
• Valid Certificate of Registration with the College of Nurses
• At least 1 year experience in related field
• BCLS certification necessary
• Membership in RNAO preferred
• Proven ability to work independently and in teams
