Join my Place Health as a Transitional Registered Nurse, focusing on comprehensive participant assessments and effective care transition planning. Work collaboratively to enhance the quality of life for older adults in California.
As a Transitional Registered Nurse at my Place Health, you will be integral in conducting thorough participant evaluations and coordinating inpatient care. With a minimum of three years of experience needed, your role includes developing transition care plans, managing authorizations, and facilitating discharge processes. Your expertise will enhance the care provided to medically complex populations.
Key Responsibilities:
• Conduct comprehensive assessments for hospitalized participants
• Coordinate care in hospitals and nursing facilities
• Develop individualized transition care plans
• Manage authorizations and hospital admissions
• Educate participants and caregivers on care strategies
Requirements:
• Active RN license in California
• Minimum 3 years experience with complex populations
• CPR/BLS certification required
• Valid driver’s license needed
• Alzheimer’s Certification preferred
