The role involves working with members unable to receive adequate support through telephonic care alone. As a clinical leader, you will perform assessments, create tailored care plans, and collaborate with various health professionals to improve patient outcomes and promote safety across healthcare settings.
Key Responsibilities:
• Perform comprehensive assessments to identify health risks
• Engage in team discussions to enhance member-centered care
• Update and implement care plans based on member priorities
• Provide coaching on disease management for members and families
• Ensure adherence to medication plans and collaborate with pharmacies
Requirements:
• CA Registered Nurse (RN) license required
• Bilingual in English/Spanish is essential
• 3+ years clinical experience preferred
• Experience with high-risk patient populations is a plus
• Strong communication and multitasking skills are critical
