Position: Alignment Health Care Anywhere Clinician
Take on a pivotal role at Alignment Health as an Advanced Practice Clinician within the Care Anywhere program, delivering compassionate care to seniors and the chronically ill r expertise will help prevent hospitalizations and ensure essential health monitoring.

This position focuses on in-home support for Alignment Healthcare patients, allowing you to conduct assessments, manage medications, and educate them about their health. Collaborating closely with a dedicated clinical team, you will tailor care plans to meet each patient's needs, making meaningful impacts on their health outcomes.

Key Responsibilities:

• Perform comprehensive in-home patient assessments

• Educate patients on care options and management

• Identify key diagnoses for ongoing medical management

• Collaborate on care plans with regional medical officers

• Coordinate care with primary and specialty providers

Requirements:

• Master's degree in Nursing or Physician Assistant studies

• Valid Nurse Practitioner or Physician Assistant license

• Preferred:
Experience in home care or geriatric settings

• Effective communication and problem-solving abilities

• Capability to lift and move objects up to 20 pounds
