What We Need As a member of the interprofessional team, the Registered Practical Nurse (RPN) in Transitional Care will work closely with the team in providing care to alternative level of care (ALC) patients awaiting transition back to the community. The RPN will practice in an environment where our focus is patient/family centered care, customer satisfaction, and ongoing professional development.
Qualifications Current Certificate of Registration with the College of Nurses
Current BLS certification
Recent experience managing patients with aggressive behaviours
Recent related inpatient rehabilitation and/or inpatient medical/surgical experience with evidence of well-developed clinical skills
RNAO membership is preferred
Excellent physical assessment skills
Proficient computer skills
Demonstrated commitment to a patient/family centered care environment
Excellent critical thinking and problem solving skills
Ability to work effectively within an interprofessional team as well as independently
Well-developed organization, communication, interpersonal, and teaching skills
Evidence of ongoing professional development, continuing education and leadership experience
Good attendance and performance records with the ability to maintain these same standards
Commitment to a healthy and safe workplace for self and others (staff, patients, families, etc.) by taking all reasonable precautions and working in compliance with organization related policies, health and safety legislation and best practices and completing relevant mandatory education as required
Work Schedule D12, N12, weekends, STATS. Please note schedules may change due to operational needs.
Compensation CU10 range: $37.64 - $39.02 per hour.
