Key Responsibilities:

• Collaborate on care planning with stakeholders

Requirements:

• Registered Practical Nurse with College of Nurses of Ontario

• Minimum one year community care experience

• Strong interpersonal and communication abilities

• Current CPR certification and valid driver’s license

• Experience with IT tools preferred
