Join Habitat Health as a Home Care Coordinator, specializing in the personalized support of older adults through comprehensive homecare. Facilitate assessments and care plans that empower individuals to thrive in their homes.
As a pivotal member of the Interdisciplinary Team (IDT), you will assess participant needs and develop tailored care plans to ensure effective delivery of homecare services. Collaborate closely with participants and their families to identify care requirements and service gaps. This role emphasizes critical thinking and strong clinical judgment to enhance participant safety and independence.
Key Responsibilities:
• Perform thorough in-home assessments and safety evaluations
• Lead the coordination of individualized home care services
• Participate in the development and updating of care plans
• Evaluate and address gaps in service documentation
• Collaborate with interdisciplinary team members for holistic care
Requirements:
• Current LVN license in applicable state
• At least two years of relevant clinical experience
• Experience in geriatrics or care coordination preferred
• Excellent organizational and communication abilities
• Valid driver’s license and reliable transportation
