As a Field Nurse Case Manager, you will be vital to Sentrex's Patient Services team, ensuring patients receive the essential support they need for their drug therapies. You’ll coordinate healthcare services and assist physicians in program enrollment, maximizing treatment outcomes through expert management and compassionate care.

Key Responsibilities:

• Manage patient and HCP registration activities

• Provide medical support to patients and providers

• Administer case-management protocols for patient follow-up

• Liaise with clinics for comprehensive patient services

• Conduct data entry and documentation of patient progress

Requirements:

• 2-4 years of clinical nursing experience

• Registered Nurse in good standing with local college

• Experience with Patient Support Programs is preferred

• Knowledge in oncology, gastro or rheumatology is an asset

• Strong organizational and communication skills
