In this role, you will be a key member of the interprofessional team, specializing in stroke care and rehabilitation. Your responsibilities will include conducting assessments, developing care plans, and educating patients throughout their recovery journey. You will play a vital part in ensuring that the care aligns with the patients' individual needs and goals.
Key Responsibilities:
• Conduct thorough patient assessments for individualized care
• Collaborate with healthcare professionals for optimal care plans
• Facilitate patient education and discharge planning
• Utilize best practices to implement nursing interventions
• Active participation in continuous quality improvement initiatives
Requirements:
• Registered with the College of Nurses of Ontario
• At least 1 year of recent rehabilitation experience
• Current BCLS and Medication Administration certifications
• Proficient in physical assessment and clinical skills
• Strong communication and teamwork abilities
