Apply advanced geriatric nursing and clinical decision-making as a Nurse Practitioner at Oak Valley Health’s Reactivation Care Centre. Work Monday to Friday, 0900 to 1700.
This Nurse Practitioner supports older adults at Markham Stouffville Hospital’s Reactivation Care Centre. The position delivers comprehensive nursing services across health promotion, prevention, treatment, rehabilitation, and support. Practice within an interprofessional team while advancing best practices in geriatric care.
Key Responsibilities:- Provide comprehensive nursing services for older adults
- Support reactivation of Alternate Level of Care patients
- Deliver direct patient care and clinical consultation
- Promote best practices in geriatric nursing care
- Collaborate with Markham Stouffville Hospital team members
- Master’s Degree and current Ontario Nurse Practitioner registration
- Five years’ current expertise in geriatric care management
- Advanced clinical knowledge and evidence-based practice implementation
- Competence in assessment, diagnosis, treatment, and health promotion
- Excellent communication, decision-making, and problem-solving skills
