Deliver community nursing care as a Registered Practical Nurse, using assessment skills and care planning during scheduled patient visits. Full-time work supports patients across various home settings.
Para Med seeks a Registered Practical Nurse for community visits involving patients of all ages and differing care complexity. You will provide care within your scope, with Registered Nurse support for more complex patients. The work emphasizes autonomy, safe practice, therapeutic relationships, and helping patients remain comfortable at home.
Key Responsibilities:- Complete scheduled community visits within your scope
- Provide ethical and safe nursing care
- Assess individual patient needs using appropriate techniques
- Monitor and adjust care plans as required
- Maintain professional patient records and encounter plans
- Register with CNO as a Registered Practical Nurse
- Maintain good standing with the CNO
- Hold a valid driver’s license and access to a vehicle
- Travel to patient residences within a designated area
- Provide required on-call coverage, including after hours
Support patients in their homes through community nursing, assessment, care planning, and safe Registered Practical Nurse practice.
