Position: Family Experience Coordinator at Sienna Senior Living

Make a difference in long-term care as a Family Experience Coordinator with Sienna Senior Living in Stouffville. Support residents and their loved ones through care coordination and relationship building.

As a temporary full-time Resident & Family Experience Coordinator, you will be a vital part of the Bloomington Cove Community team. Engage with residents and families to enhance their daily experiences while promoting service excellence.

Your role involves leading care coordination and resolving concerns to maintain satisfaction for all parties involved.

Key Responsibilities:

• Coordinate resident move-in, transfer, and move-out processes

• Provide social service support to enhance resident wellness

• Lead family satisfaction surveys and implement action plans

• Facilitate resident care conferences for ongoing support

• Analyze and report on resident and family feedback quarterly

Requirements:

• College/University degree with Social Service Worker designation

• Minimum 3 years in healthcare or social services

• Detail-oriented with strong customer focus

• Ability to multitask in a dynamic environment

• Tactful and diplomatic in interactions with seniors
