Position: Part Time Social Worker Role

As a Social Worker, you will engage with a diverse patient caseload, providing critical support in acute care settings. Collaborating with an interdisciplinary team, you will ensure patients receive the necessary support during difficult times, including end-of-life care.

Key Responsibilities:

• Complete psychosocial assessments for diverse patients

• Provide focused counseling for grief and adjustment issues

• Develop and implement patient-centric care plans

• Support fellow Social Workers through cross coverage

• Engage with community health professionals for resource access

Requirements:

• Master's degree in Social Work

• Registered with Ontario College of Social Workers

• Experience in hospital settings
