Center Well Home Health is seeking a licensed Physical Therapist to provide in-home therapy, helping patients regain strength, mobility and independence through personalized plans of care. Reporting to the Clinical Manager, you will collaborate with a multidisciplinary team, document progress, and ensure high-quality, patient-centered rehabilitation in patients' homes.
This role requires CPR certification, a current PT license, and a willingness to participate in Humana’s TB and driver-safety
