Hospice RN Case Manager On Call (Weekdays)

RN On Call Nurse (Weekdays) position focused on providing hospice nursing care with responsibility for assessing, planning, implementing and evaluating the nursing process, including pain control and symptom management. This role requires collaboration with physicians, hospice staff, and other agencies to coordinate care for patients and families.

• Assumes responsibility for a patient visit or on-call schedule and participates in the nursing process (assess, plan, implement, evaluate).
• Provides hospice nursing care based on systematic assessment, focusing on pain control and symptom management.
• Initiates communication with attending physicians, other hospice staff, and other agencies as needed to coordinate care and resources for the patient/family.
• Maintains regular communication with direct supervisor to review assignments.
• Maintains up-to-date documentation at point of service so problems, plans, actions, goals and charges are accurately recorded in accordance with agency policy and Medicare regulations.
• Administers medications in compliance with federal and state laws and hospice policies.
• Maintains current patient records and documents in accordance with Ascend Hospice & Palliative Care MA policies; ensures problems, interventions, goals and outcomes are accurately stated and charges are reflected.
• Assesses and collaborates with Medical Director to determine initial and ongoing eligibility of the hospice patient.
• Completes initial assessment of patient and family to determine home care needs.
• Anticipates end-stage disease process and related pain and symptom management; regularly re-evaluates nursing needs and uses assessment data to determine care plan needs.
• Initiates appropriate preventive and palliative nursing procedures.
• Counsels the patient and family in meeting end-of-life needs; performs a comprehensive death note when appropriate.
• Communicates with the physician regarding the patient’s needs and reports any changes in condition; coordinates with the IDT to align the care plan.
• Participates in on-call duties as defined by the on-call policy.
• Ensures arrangements for equipment and other necessary items and services are made as needed.

Education: Associate's Degree in Nursing

• Must have a State Certified RN license
• Experience in hospice or home health care, REQUIRED
• Excellent documentation skills and ability to work independently
• Strong communication skills
• Valid driver’s license and reliable transportation
• Must be available to travel locally within assigned territory
