Licensed Social Worker – Wiley Christian Retirement Community (Moorestown, NJ)

Part‑time position (up to 16 hours per week). Wiley Mission has earned the reputation for quality of care, courtesy and professionalism. The Social Services department is part of a multidisciplinary team that meets the psychosocial needs of residents and supports overall quality of life across care levels.

• Identify and support the social and emotional needs of residents.
• Collaborate with other members of the multidisciplinary team to implement a coordinated approach to providing the highest quality care.
• Perform the advocacy role and represent residents to the multidisciplinary team and families, as appropriate.
• Coordinate discharge services for residents (personal care in‑house services, meals, transportation, respite care, adult day care, support groups).
• Participate in care planning and care team meetings, acting as an integral member of the interdisciplinary team responsible for overall well‑being of residents.

• Licensed Social Worker in the State of New Jersey.
• Three years of experience in a multi‑level long‑term care setting.
• Subacute experience a plus.
• Working knowledge of State and Federal requirements related to long‑term care.
• Effective and professional communicator with older adults and their families.
