Responsibilities- Identify the targeted population and implement clinical interventions and protocols.
- Collaborate with primary care providers, patients, and all members of the health‑care team across continuity of care settings and community resources.
- Provide self‑management support and include patients/families as active members of the team approach.
- Facilitate close monitoring of high‑risk patients to prevent and/or intervene early during acute exacerbations.
- Coordinate care between primary care providers and acute‑care hospitals, specialists, and post‑acute‑care services.

Applicants must hold a Michigan RN license and possess an associate degree from an accredited nursing school (bachelor’s degree preferred).
- Basic Life Support Health‑Care Provider (BLS‑HCP)
- Minimum two (2) years of experience in adult medicine and pediatric patients.
