The LPN is a member of a healthcare team, which includes a provider, behavioral health consultant, registered nurse (RN), medical assistant, care coordinator, and patient service representative (PSR), all assigned a panel of patients.
Working within the full scope of LPN licensure, the LPN provides direct nursing care under the immediate supervision of a registered nurse, physician, advanced practice nurse, or physician assistant. Responsibilities include, but are not limited to, obtaining and recording vital signs, discussing chief health concerns, performing, documenting, and following up on laboratory tests, assisting with specialized procedures, performing tests (vision, peak flow, lung function, EKGs, etc.),
obtaining samples for lab tests through venipuncture, nasal swabbing, etc., administering and charting medications, and performing injections including immunizations. The LPN must maintain education requirements to sustain certification and attend continued education as required by supervisor, state, or federal guidelines. LPN coverage at other locations may be required.
Minimum Qualifications:
- Possess a licensed practical nurse license.
- Possess current CPR certification.
- Willingness and desire to lead and mentor MAs and LPNs.
- Ability and means to travel on a flexible schedule as needed; proof of liability and property damage insurance on vehicle used is required.
