We are seeking a Speech Language Pathologist passionate about improving lives! In this role, you will evaluate, develop care plans, and deliver treatment for swallowing, dysphagia, and communication disorders, primarily for adult and geriatric patients, following state guidelines.
Key Responsibilities- Conduct screenings and evaluations to assess patients' needs for skilled speech therapy, focusing on swallowing and communication disorders.
- Create and implement treatment plans targeting functional outcomes based on physicians' directives and professional standards.
- Monitor patient progress and modify treatment plans as necessary.
- Maintain accurate patient records.
- Collaborate with community staff, patients, families, and rehabilitation teams through training and coordination.
- Engage in quality improvement activities.
- Completion of an accredited Speech Language Pathology program.
- Current valid license to practice as a Speech Language Pathologist in the relevant state.
- Excellent communication, interpersonal, problem-solving, and critical thinking skills.
- Ability to work independently and collaboratively.
