MDS Coordinator

Join our friendly, caring and supportive team at Legacy Healthcare LLC. We are currently searching for an MDS Coordinator to ensure high standards of care for each resident.

• Develop, monitor, audit, and modify each resident’s care plan.
• Perform resident assessments and assist in the discharge process.
• Ensure residents receive the high standard of care expected at Harmony Marshalltown.

• State of Iowa RN or LPN license (unencumbered).
• Accredited nursing program graduate and CPR certification.
• Preferred: 1 year RN in a Skilled Nursing Facility and 1 year CNA experience, but we are willing to invest in the right candidate.
