PHC is hiring a Registered Nurse - Nurse Care Manager to join our Ames/Marshalltown area team. You will implement risk stratification, manage chronic conditions, and provide holistic care coordination for patients with complex needs within an interdisciplinary team.
Expect standard weekday hours 8 AM–5 PM with occasional travel between PHC clinics. Active RN license in Iowa, BLS, and strong communication and problem-solving skills are required.
