Wage: $57,709.60 to $65,286 annually. Wage will increase on 7/1/26 to $58,858.15 – $66,595.75, with another increase on 1/1/27 based on a satisfactory performance review.
GENERAL FUNCTION
Assess, plan, and coordinate needed support and services for children with physical, cognitive, and emotional disabilities. Service Coordinators also support the immediate family members, helping to resolve issues that arise. The role enables Wood County children to receive the items and services they need to stay safe and remain in their homes and community, rather than being placed in more restrictive environments.
RESPONSIBILITIES
ESSENTIAL FUNCTIONS
Conduct assessments and develop and implement Individual Service Plans for children in the Children’s Community Options Program (CCOP) and the Children’s Long-Term Support (CLTS) Medicaid Waivers Program.Provide direct supportive services to children and families on caseload including information/referral, supportive counseling, assistance in making applications for financial assistance programs, etc.Conduct home visits and/or set up virtual video visits to discuss family issues/concerns and document all relevant information within CCOP and CLTS Medicaid Waivers Program.Complete Functional Screens (initial, annual, updates). Update Children’s Functional Screens annually.Complete paperwork other than Functional Screens (case notes, letters, emails, faxes, funding requests/authorizations, WPS authorizations, Individual Service Plans (ISP), Outcomes, referrals, review of medical records, review of Individual Education Plans (IEP), review of parents/guardians requests of items/services, incident reports, data entry into Forward Health Portal, high-cost funding requests, parental fee calculations, other state and county forms, assessments, etc.).Make home visits to conduct intakes, reviews, check-ins, provide support, attend school meetings and/or medical appointments, etc.Make telephone calls to clients, providers, state contacts, therapists, evaluators, contractors, and other professionals and non-professionals associated with the provisions of the County operated services.Create and help manage CLTS Waiver Third Party Administration (TPA) authorizations with the Business office staff.Perform all duties in compliance with the Medicaid Home and Community-Based Services (HCBS) Waiver Manual for the CLTS Waiver Program and standards set out in Medicaid Targeted Case Management regulations.Manage the expenditures of CCOP funds, submit timely vouchers, and work closely with the Business office staff in completing state-required reporting systems (Human Services Reporting System and Program Participation System).Know and practice Wood County safety policies.
