Licensed Practical Nurse - Part-time / Full-time
All Care Homecare is seeking to hire a full-time & part-time

Responsibilities- Deliver professional nursing services in client’s home setting and provide case management for primary patients
- Complete initial assessment and plan of care for home care clients
- Prepare clinical and progress notes, submitting all required paperwork daily
- Coordinate services, informing the physician, management, and other personnel of changes in the client’s condition and needs
- Ensure quality and accuracy of patient records and charts in compliance with Medicare and Medicaid standards
- Demonstrate compassion while working with ill, disabled, elderly and/or emotionally disturbed patients
- Respond promptly to telephone calls and emails
- Participate in continuing education programs to stay current on home health practices, laws and regulations
- Post hospital visits
- Monthly client visits
- Help maintain confidentiality files
- Medication management services
- Perform other duties as assigned
- RN with current MA license, BSN preferred
- One or more years of current nursing experience in certified home care is preferred, but not required
- Ability to sit, stand, bend, lift, kneel, climb stairs and move patients safely as required
- Ability to work in the patient’s home environment as well as the office
- Demonstrate strong verbal, written and interpersonal skills to communicate effectively
- Must have reliable automobile, valid driver's license, and current auto insurance
- Fluent in English; skills in a second language a plus
- Computer skills sufficient to capture notes and required documentation
