Licensed Practical Nurse - Part-time / Full-time

All Care Homecare is seeking to hire a full-time & part-time

• Deliver professional nursing services in client’s home setting and provide case management for primary patients
• Complete initial assessment and plan of care for home care clients
• Prepare clinical and progress notes, submitting all required paperwork daily
• Coordinate services, informing the physician, management, and other personnel of changes in the client’s condition and needs
• Ensure quality and accuracy of patient records and charts in compliance with Medicare and Medicaid standards
• Demonstrate compassion while working with ill, disabled, elderly and/or emotionally disturbed patients
• Respond promptly to telephone calls and emails
• Participate in continuing education programs to stay current on home health practices, laws and regulations
• Post hospital visits
• Monthly client visits
• Help maintain confidentiality files
• Medication management services
• Perform other duties as assigned

• RN with current MA license, BSN preferred
• One or more years of current nursing experience in certified home care is preferred, but not required
• Ability to sit, stand, bend, lift, kneel, climb stairs and move patients safely as required
• Ability to work in the patient’s home environment as well as the office
• Demonstrate strong verbal, written and interpersonal skills to communicate effectively
• Must have reliable automobile, valid driver's license, and current auto insurance
• Fluent in English; skills in a second language a plus
• Computer skills sufficient to capture notes and required documentation
