Youth Services Mentor-After School

Responsibilities
  • Demonstrated ability to teach others, including Daily Living Skills, social skills, and community involvement.
  • Assist and support children and youth with disabilities in becoming actively involved in their communities, developing friendships, and using community resources independently.
  • Regularly provide opportunities for inclusion through planned activities, social interactions, exposure, advocacy, training, and education as identified in the assessment and planning process.
  • Provide assistance with personal care tasks, as needed (mobility, eating, bathroom, etc.).
  • Be knowledgeable in and able to document properly on all individual services provided daily in the required time frame and format.
  • Maintain open, respectful, and clear communication with the participant, their families, and supervisor.
  • Meet all necessary training requirements.
  • Transport and accompany individuals to community activities in company or personal vehicles.
  • Must pass WI caregiver background check.
  • Hold a valid Driver's License.
  • Work flexible/non‑standard schedule (including some evenings and weekends).
  • Use a company email address and apply for MFA on personal cell phone.
  • Have a reliable vehicle (unless exempt with HR).
Qualifications
  • Minimum of 1 year work experience with individuals and/or children with disabilities.
  • Excellent written and verbal communication skills.
  • Knowledge in modifications, adaptation, and use of specialized equipment.
  • General knowledge of computer use and programs required.
