ILS Case Manager - Independent Living Services
Full‑time salaried/exempt management position responsible for supporting the delivery of high‑quality, person‑centered Independent Living Skills services to adults with intellectual and developmental disabilities (I/DD).
Key Responsibilities- Partner with the ILS Program Manager to oversee service coordination, compliance, and consumer independence, safety, and community integration.
- Dedicate at least 51% of work time to management and administrative duties (oversight, coordination, documentation, compliance) and no more than 49% to direct client support.
- Conduct case management: support clients with lifestyle planning, individualized Support Plan (ISP) development and monitoring, daily progress documentation, and regular care plan reassessment.
- Advocate for clients during Interdisciplinary Team (IDT) meetings, facilitate community networking for natural supports, and initiate and co‑facilitate Circle of Support gatherings.
- Supervise ILS Instructors: participate in hiring, training, performance evaluations, staffing schedules, time‑card approval, and OSHA‑compliant safety supervision.
- Design and deliver comprehensive staff training, provide ILS Title 17 training to consumers, and conduct ongoing educational sessions for team members.
- Education: BA/BS in a human services delivery field.
- Experience: Minimum two years working with developmentally disabled adults; ILS or special education teaching experience preferred. Combination of education and experience may substitute for part of the qualifications.
- Transportation: Valid California driver’s license, clean DMV record with proof, current vehicle registration, a vehicle in good running condition for work hours, and valid auto insurance meeting agency minimum coverage.
- Core Competencies: Strong verbal and written communication, conflict mediation, ability to conduct effective interviews, strong record‑keeping, ability to analyze complex situations, and ability to manage multiple priorities independently.
- Other requirements:
Tuberculosis clearance screen, pass a criminal background check with fingerprint clearance, willingness to perform all essential functions, and an employment record of punctuality, good attendance, and reliability. - Must provide documents required for an I‑9 application to prove eligibility to work in the U.S.
- State and federal laws related to Adult Protection, Developmental Disabilities, and services for Vulnerable Adults.
- Social work principles, methods, and techniques.
- Employee supervision and management principles.
- Adult education and methodologies. Personal healthcare practices and principles.
- Home management, nutrition, meal planning, aging process, and related behavioral changes.
Regular travel, prolonged driving, and ability to transition between office and client homes. Must sit, stand, walk for extended periods, occasionally lift up to 50 pounds, and safely drive a passenger vehicle. Work occurs in both office settings and client homes, with variable weather and noise levels.
