What You’ll Do- Deliver skilled nursing care under the direction of a Clinical Case Manager
- Administer medications and treatments as prescribed
- Monitor and document patient health status and progress
- Support patients and families with education and care guidance
- Collaborate with the care team to ensure safe and effective treatment

- Previous experience in home health, long-term care, or clinical setting preferred but not required
