• Active RN license in the state of Virginia
• Minimum 2 years of recent experience as a Hospice Registered Nurse
• Proficiency with OASIS and HCHB (Homecare Homebase) documentation systems
• Current CPR certification
• Valid driver’s license, vehicle insurance, and reliable transportation

• Serve as a Hospice Case Manager, delivering patient-centered care in home health settings
• Conduct comprehensive patient assessments and develop individualized hospice care plans
• Document patient progress accurately using OASIS and HCHB systems
• Educate patients and families on hospice care, symptom management, and available healthcare resources
• Collaborate with physicians, social workers, and interdisciplinary healthcare professionals to ensure quality patient care
• Participate in on-call rotation as required
