Moms In Motion is seeking a Service Facilitator (SF) Case Manager who will travel locally to visit clients in their homes across Virginia. You’ll guide families through Medicaid Waiver programs (CCC+, CL & FIS, EPSDT) and develop plans of care to support safe, healthy, independent living.
This role requires no degree, at least 2 years of relevant experience, and a valid driver’s license. Strong communication and organizational skills are essential; a background check and references are required.
