Position Summary

The Home Health Speech Therapist (SLP) provides evaluation and speech‑language pathology treatment in accordance with agency standards, state laws, and other regulatory requirements. The role is 40 hours per week.

Responsibilities
  • Arrange specialized assessments that cannot be conducted in the home (e.g., audiological testing, videofluoroscopy) and plan discharge with referrals to community resources.
  • Communicate with a supervisor as scheduled and as needed regarding patient care and service delivery.
  • Contribute to departmental goals, adhere to policies, procedures, quality standards, and safety standards; complete mandatory training (Code of Conduct, Compliance, etc.) and comply with governmental and accreditation regulations.
  • Maintain effective working relationships with other departments and participate in multidisciplinary quality and service improvement teams, meetings, and committees.
  • Maintain ongoing communication with the patient’s physician, health care team, case managers, and referring facility personnel regarding aspects of care and patient status.
  • Perform other duties as assigned.
  • Provide assessment, in collaboration with the health care team, of the patient’s environment and recommend adaptive equipment or home modifications to maximize function and safety; assist in obtaining reimbursable durable medical equipment.
  • Conduct initial and periodic assessments to determine habilitation/rehabilitation needs; establish and modify an effective treatment plan with the patient, caregiver, physician, and team; perform evaluations including interview, observation, and specialized assessments (dysphasia, oral motor, receptive/expressive language). Assessments may require tools such as a sphygmomanometer, stethoscope, and appropriate focal senses.
  • Provide medical record entries per agency standards, professional guidelines, and regulatory requirements for reimbursement.
  • Educate patients and caregivers about the injury/disease process, functional limitations, and treatment purpose; train caregivers on post‑discharge care.
  • Implement speech‑language pathology/dysphasia services per the treatment plan, agency standards, and professional guidelines; perform procedures and modalities such as associative language exercises, oral motor exercises, aural rehab, and thermal stimulation.
  • Supervise home health aides in patient care and related activities.
Qualifications
  • 1‑2 years of clinical experience (required)
  • Master’s degree in Speech‑Language Pathology (required)
  • License to practice Speech‑Language Pathology in the State of Maryland (required)
  • CPR certification (required)
  • Valid driver’s license with an acceptable driving record and reliable transportation (required)
  • Strong clinical skills
  • Excellent verbal and written communication skills
  • Organizational skills
  • Basic computer skills (preferred)
  • Smartphone experience
Schedule
  • Monday – Friday; 8:00 am – 4:30 pm with some weekend rotation
Compensation
  • $113,000 – $150,000 per year
  • Mileage reimbursement at IRS rate
Work Location
  • Out in the field providing patient visits (home)
Coverage Area
  • St. Mary’s County – assigned a centralized territory to reduce windshield time
Productivity Requirements
  • 27 points per week:
    Admission/recert/resumption = 2 points;
    Revisit/discharge = 1 point;
    Additional clinical eval = 1.5 points
