Coordinator

The Intake/Admission Coordinator serves as the first point of contact for patients, families, and referral sources seeking hospice or palliative care services

The primary responsibility of this role is to ensure that our patients experience the benefits and beauty of hospice services, and this is possible when the Intake/Admissions Coordinator consistently uses active listening, and offers help based on identified needs. Each interaction should be handled with compassion, empathy and efficiency, all with the goal of optimizing comfort and quality of life.

Position Summary:

The Intake/Admissions Coordinator is responsible for managing the intake and admission process for patients electing hospice services. This role involves coordinating with patients, families, and healthcare providers to ensure a smooth and compassionate transition into hospice care. The coordinator will manage non-clinical incoming calls, support referral coordination, and maintain accurate records.

The Intake/Admissions Coordinator is responsible for managing the intake process for patients seeking palliative care by verifying referral information, collecting relevant patient data, and initiating the referral workflow which is then passed on to the Palliative Care Manager.

Director of Clinical Services

Responsibilities & Functions:
  • Must possess a strong and proven commitment to ensure that every caller receives excellent service, every time they contact Compass.
  • Obtain patient history relevant to patient admission to include insurance verification.
  • Immediately input the caller’s identifying information, contact information, and needs into the EMR
  • Collaborate with other team members to respond quickly to the needs of the patient, with the goal of bringing hospice-eligible patients into service timely
  • Coordinates with the Admissions Nurses or Clinical Director to ensure delivery of hospice services.
  • Provide emotional support to patients and their families.
  • Ensure services are provided in accordance with federal, state, and agency regulations.
  • Facilitate communication between patients, their families, and the healthcare team.
  • Prepare admission packets.
  • Coordinate admissions in a timely manner according to Compass policy and protocol.
  • Contact Durable Medical Equipment (DME) companies for delivery of equipment when backup to team is needed.
  • Refer patients to other programs offered by Compass when appropriate.
  • Maintains required HIPAA confidentiality of employees, patient/client, and agency information.
  • Utilize volunteers for reduction of workload.
  • Accepts and performs other related duties and responsibilities as required.
Requirements:
  • Proficient in completing tasks efficiently and accurately.
  • Able to manage sensitive and confidential information appropriately.
  • Strong people skills, including communication, patience, and compassion.
  • Excellent oral and written communication skills.
  • Professional demeanor, able to maintain composure during challenging situations.
  • Accountable and able to take ownership of tasks and responsibilities.
  • Experience with Electronic Medical Records (EMR) systems.
  • Organized and able to multitask effectively.
  • Patient advocate, providing support to ensure desired and required care.
  • Problem-solving and critical thinking skills.
  • Support the hospice concept of care.
  • Ability to work with diverse individuals, teams, and community groups.
  • Willingness to participate in continuing education, in-service training, and workshops required for all Compass staff.
  • An ability to cope with the stress of continual loss.
  • Must be able to lift 15-20 pounds.
  • Must be able to hear instructions in person and by phone.
Support & Training:
  • Assist with the job orientation for new employees.
  • Assist with staffing needs when required.
