Overview
The Family Medicine Nurse Practitioner at Greater Baden Medical Services (GBMS) is responsible for providing direct patient care and care consultation for a panel of patients as well as patients s position requires strong family medicine patient care experience. The Family Medicine Nurse Practitioner practices in a team-based care model and collaborates with other members of the team to care for a panel and a population of patients.
The Family Medicine Nurse Practitioner refers patients for medical care management and care coordination services with internal staff and/or with other agencies. The Family Medicine Nurse Practitioner is responsible for timely follow up of specialty referrals and diagnostic testing. The Family Practice Nurse Practitioner prioritizes meeting and exceeding quality care measures as identified by GBMS.

Essential Functions- Provides comprehensive physical assessment of adult patients and evaluates the outcome of patient care.
- Prescribes or recommends forms of treatment such as pharmaceutical therapy, physical therapy, inhalation therapy, or related therapeutic procedures.
- Conducts age-appropriate risk assessments and developmental assessments.
- Assures quality of patient care by participating in the organization’s Performance and Quality Improvement activities.
- Advocates and orders immunizations according to current ACIP and/or CDC guidelines.
- Arranges referrals, consultations, therapeutic services, and confers with other specialists on course of care and treatment.
- Establishes medical diagnoses for common short-term and chronic stable health problems.
- Orders, performs, and interprets laboratory and diagnostic tests.
- Orders and performs diagnostic, therapeutic, and corrective measures; prescribes drugs.
- Provides same-day care as required.
- Refers patients to appropriate licensed physicians or other health care providers as needed.
- May refer patients to physicians for consultation or to specialized health resources for treatment.
- Provides direct patient care as needed and appropriate to serve the community in accordance with the organization’s mission, vision, and values.
- Provides documentation of clinical care in a timely manner, including maintenance of patient charts in accordance with clinic protocols and timely documentation of procedures on encounter forms.
- Complies with regulations regarding consultations and other services as required by patients’ insurance coverage.
- Complies with Joint Commission standards for ambulatory care and with Patient-Centered Medical Home standards and guidelines.
- Performs timely entry into and management of the Electronic Health Record.
- Completes chart documentation timely in accordance with organizational policy.
- Complies with the organization’s training requirements and risk management programs.
- Provides clinical leadership and acts as a resource to other members of the health care team, patients, or community members.
- Provides clinical support and acts as a resource to the Chief Medical Officer and other providers.
- Works with center manager to facilitate smooth operation of clinic flow.
- Cooperates with compliance review activities.
- Submits information needed for credentialing files in a timely manner.
- Participates in team/staff meetings; attends in-service meetings and applicable workshops.
- Performs other duties as required to provide optimal patient care.
- CMAs, fellows, students, interns and the like if applicable.
- Current License to practice medicine in the State of Maryland.
- Current Maryland prescriber number, and current D.E.A. and C.D.S. License.
- Nurse Practitioner Board certification preferred.
- Current Cardiopulmonary Resuscitation Certificate.
