Stella Maris is looking for Home Health RN Case Managers to join their growing Home Health team. This is an exciting opportunity to work alongside dedicated clinicians to meet the needs of clients in the community we serve.

• Under the direction of the clinical manager, perform primary nursing care functions in conformance with professional standards of home health nursing practice.
• Provide individualized care to patients in their homes according to the plan of care approved by the primary physician.
• Coordinate all services provided for each assigned patient and family.
• Provide leadership and advocacy in the coordination of patient care with multiple clinicians, modalities, and community resources.
• Manage clinical resources and transition planning for patients within the assigned caseload from pre-admission through post-discharge, actively working to provide excellent medical care.
• Travel daily within the assigned region.

• RN licensure in the State of Maryland required.
• Prior experience in hospice or home health strongly preferred.
• Valid driver’s license and reliable transportation.
