Weekend Admissions Nurse (RN) - Home Health

Shift: Friday, Saturday, Sunday 8:00 AM – 6:30 PM.

• Main point of contact to answer admissions questions about home health services to prospective patients and families.
• Explain insurance benefits and coverage, patient responsibilities, and alternative payment options.
• Coordinate the admissions process to the appropriate program and complete paperwork promptly and accurately.
• Develop the initial plan of care based on input from patient, family and physician.
• Provide information and education to physicians, discharge planners, and community organizations about Stella Maris Home Health services.

• RN licensure in the State of Maryland.
• Prior experience in home health required.
• Valid driver’s license and reliable transportation.
• OASIS experience required.
• This position requires daily travel in the assigned region (Baltimore County & Baltimore City).
