Wellness Home Services Licensed Practical Nurse (LPN)

Join our team at Acts Retirement-Life Communities as a Licensed Practical Nurse (LPN) and help deliver individualized, one‑on‑one care to our residents. Working under the direction of a Registered Nurse (RN) and in compliance with state guidelines, you will manage chronic conditions, support post‑operative recovery, and address complex medical needs while ensuring safety, dignity, and comfort.

• Provide medication management and vital sign monitoring.
• Collaborate with multidisciplinary care teams to develop and implement care plans.
• Offer education and guidance to residents and families regarding health conditions and care procedures.
• Support residents in maintaining independence and improving quality of life.

• Active LPN license in good standing with the applicable state Board of Nursing.
• Minimum 1–2 years of clinical nursing experience, preferably in acute care or home health.
• Current CPR certification (or obtaining within three months of employment).
• Valid state driver’s license and proof of current automobile liability insurance if transportation of residents is required.

$24.38 – $35.79 per hour. Starting rate will vary based on skills and experience.
