Personalized Health Partners is seeking a Nurse Practitioner to deliver high-quality primary care for residents at South River Healthcare Center. Schedule is Monday–Friday with flexible daytime hours and no weekends or on-call; telehealth may be utilized where appropriate.

The NP will diagnose, treat, and manage acute and chronic conditions, collaborate with PCPs and interdisciplinary teams, and engage residents and families in care planning to improve outcomes and satisfaction.
