Healthy Families/Parents as Teachers (PAT) Hybrid Program Family Support Worker/Family Assessment Worker (12-month, Full-time)
LocationOffice:
Plum Point Elementary School, and assigned home visits.
Responsible for initiating and maintaining regular, long‑term (up to three years) contact and support with families, primarily within the family’s home, seeking at least one hour per visit. Services are family‑centered, strength‑based, and directed at establishing a trusting relationship; assisting parents in strengthening the parent‑child relationship; improving the home environment; increasing family support systems; and increasing families’ problem‑solving and advocate roles.
Make appropriate referrals to community programs and agencies, follow up to ensure services are sought and received, and assist families in establishing goals and a success plan for the target child.
Conduct the Parent Survey with prenatal women and parents of newborns to determine eligibility; refer families during the Parent Survey visit when appropriate; perform community outreach; maintain referral data for reporting purposes.
Reports ToProgram Coordinator or Grant Manager.
Education, Certification, and Experience- Bachelor’s degree in health, education, or human services (or related field); 3 years of relevant experience.
- Experience working with children and families preferred, including home visitation experience.
- Verbal, written, and communication skills.
- Access to a dependable vehicle.
- Demonstrated success in accomplishing tasks related to responsibilities.
- Initiate contacts and referrals to the Healthy Families program.
- Promote the Healthy Families program within the community to encourage appropriate referrals.
- Maintain regular family contact and provide parenting support; defined by level as assigned.
- Establish Family Support Plan for active families and update plan(s) in established time frames.
- Apply basic counseling skills; develop rapport, establish supportive relationships, and maintain professional boundaries with active listening and empowerment techniques.
- Demonstrate motivation to apply knowledge of child and family development; use curriculum and other resources to plan appropriate parent/child activities.
- Assist parents with arranging medical and other appointments.
- Know community resources; make appropriate referrals and act as liaison to other agencies.
- Maintain casework standards; documentation completed within two working days of service date.
- Participate in approved continuing education training.
- Experience in working with culturally diverse families; culturally sensitive and non‑judgmental.
- Interact with families from a strength‑based model even in chaotic environments.
- Advocate for nurturing, nonviolent discipline of children.
- Maintain dependable, punctual work schedule; approvals required for late afternoon, evening, or weekend appointments.
- Perform other duties as assigned (team management meetings, client resource development, etc.).
- Comply with all HFCC policies and procedures (regulations, standards, Code of Conduct).
- Keep supervisor informed of activities, issues, events, and potential problems; effective oral and written communication skills.
- Support program goals of Healthy Families Maryland and Healthy Families America.
- Treat clients, public, and staff with courtesy, respect, and dignity; positive professional image.
- Attend and participate in staff meetings; work with others to accomplish program goals.
- Think, concentrate, and interact positively with others.
- Arrive promptly every day; flexible hours as necessary; work under stress and meet all deadlines.

Terms of EmploymentTwelve‑month position; $21.64 per hour; 35 hours per week; contingent upon continued grant funding.
