Med Star Health’s Washington Hospital Center is seeking an LMSW/LMGW Inpatient Case Manager to join our Case Management Team. You will facilitate quality, cost-effective, patient-centered care from pre-admission through post-discharge and coordinate the plan of care with the family and care team.

The ideal candidate will have a master’s degree in social work and a licensed social worker credential, with hospital-based case management experience preferred.
