$22.00/hr - $29.00/hr

The Wraparound Facilitator works full‑time out of our Yuba City office and facilitates a family‑centered, strength‑based, culturally sensitive planning process for children, youth, and families. The position includes Intensive Care Coordination (ICC) and supports the provision of mental health services.

• Facilitate and participate in Katie
  
  A. Child and Family Team (CFT) meetings.
• Provide Intensive Care Coordination (ICC) that facilitates assessment, care planning, and coordination of services, including urgent services.
• Ensure the provision of Intensive Home‑Based Services (IHBS).
• Incorporate Strengthening Families Approach, the Five Protective Factors Framework, Trauma‑Informed Service Delivery, Nurtured Heart Approach, and Safety‑Organized Practice throughout the Wraparound process.
• Ensure clients receive access to mental health services, including assessment and individualized treatment that creates a coherent, all‑inclusive approach to service planning and delivery.

• A Bachelor’s degree in health, human services, or a related field from an accredited college or university.
• At least three (3) years of experience working with children, adolescents, and families in an educational or child‑development setting.
• Bilingual experience is a bonus but not required.
• Possession of a valid State of California Driver License in good standing.
