Clinical Pharmacist

Clinical Pharmacist – Inpatient Pharmacy at Memorial Health Ohio. Clinically progressive, forward-thinking pharmacy department is recruiting for a full-time inpatient care clinical pharmacist with advanced practice responsibilities across hospital service lines. The role includes development and implementation of pharmacy policies and procedures; developing agendas and presenting recommendations to the Pharmacy & Therapeutics Committee; maintaining IV guidelines and other therapeutic guidelines; and developing and implementing changes to medication order sets within the EMR by committee, among other advanced pharmacy practices.

• Provide inpatient clinical pharmacy services across hospital service lines with advanced practice responsibilities.
• Assist in developing and implementing pharmacy policies and procedures; develop agendas and present recommendations to the Pharmacy & Therapeutics Committee.
• Maintain IV guidelines and other therapeutic guidelines; develop and implement changes to medication order sets within the EMR by committee.
• Utilize clinical training to support patient care; collaborate with care teams; communicate effectively with patients and staff.
• Support the department's mission by leveraging advanced technologies and the sterile compounding suite (USP 797/800) and related workflows.

• Must be eligible for and maintain licensure as a pharmacist in Ohio.
• 2–5 years of relevant clinical pharmacy experience (particularly inpatient care) and/or completion of a clinical practice residency preferred.
