We are looking for someone to join our Specialty Care Center team as an Licensed Practical Nurse! We look forward to seeing your application.

• Performs delegated nursing functions under the supervision of the Registered Nurse using established procedures, policies, guidelines, and standards; collaborates with other health care providers to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes.
• Manually cleans and disinfects patient care items and equipment; notifies director of missing or malfunctioning equipment.
• Answers phones, records messages; greets patients/visitors; prepares charts for physician clinics; schedules patient appointments; obtains and documents all relevant patient information necessary for scheduling patient; obtains insurance pre-authorization when necessary.
• Documents and reports observations to the Registered Nurse or Provider; accurately documents patient care provided on appropriate hospital forms, and within the established time frames. Provide patient/significant other health teaching as delegated by and observed by the Registered Nurse. Participates in identification of clinical or operational performance improvement opportunities and assists in performance improvement activities that demonstrate positive outcomes in patient care.

• Licensed as an LPN in the State of Ohio.
• Completes twenty-four (24) contact hours of approved CE during each two (2) years licensure renewal period consistent with requirements of Ohio Board of Nursing.
• Certified in IV therapy.
• Attains within 60 days and maintains continuous certification in American Heart Association BLS.
