MDS RN Nurse

MDS Nurse (RN or LPN) – Full Time

Skilled Nursing Facility

We are seeking a Full-Time MDS Nurse (RN or LPN) to oversee and complete accurate MDS assessments in compliance with CMS and PDPM requirements
. This role works closely with the interdisciplinary team to ensure timely assessments, accurate reimbursement, and quality resident outcomes.

Responsibilities:

• Complete and coordinate MDS assessments per PDPM guidelines

• Ensure compliance with CMS, Medicare, and Medicaid regulations

• Collaborate with nursing, therapy, and IDT

• Maintain accurate documentation and submission timelines

• Assist with audits and survey readiness

Qualifications:

• Active RN or LPN license

• MDS experience in SNF/LTC preferred

• Knowledge of PDPM and RAI process

• Strong organization and communication skills
