NURSE PRACTITIONER - PHYSICIAN ASSISTANT

- Full orPart Time (Minimum of 4 Hours a Day)
- Hours of Operation: 8:00AM-6:00PM (Schedule must be set within these hours)

We are currently seeking a Nurse Practitioner or Physician Assistant tomanage patients in skilled nursing and long-term care facilities. The incoming provider will commit to the highest quality of care using an organized,multidisciplinary team approach to provide continuity of care throughout thepost-acute continuum. Our patient population served includes adults and geriatrics.
Responsibilities- Embrace the philosophy of our company to render care in amanner, approach and with the skill that promotes maintenance or enhancement ofeach patient's clinical status and quality of life.
- Conduct Annual Wellness Visits on all long-term Nursing Facility residents and may perform Annual History and Physicals as requested by Facilities.
- Conduct clinical interventional and episodic direct care visits, and appropriate re-evaluation of care visits.
- Provide ongoing evaluation and treatment of chronic,co-morbid illnesses, as medically appropriate
- Evaluate/treat residents for changes in condition.
- Review medications, orders, and diagnostic tests, and respondto clinical abnormalities appropriately for all patients under their care, asallowed and/or requested by collaborating physicians.
- Coordinate care with collaborating physicians as indicatedfor overall medical management.
- Assist with facility-wide illnesses, where appropriatecollaborations exist.
- Coordinate care with other disciplines, such as Therapists,Specialists, and Social Workers.
- Maintain communication with families/attend family conferences when possible.
- Other duties as assigned
- Valid Iowa Nurse Practitioner or Physician Assistant License
- Bachelors and Masters Degree in Nursing or Physician Assistant Program from accredited Colleges or Universities.
- Experience in nursing is required, and some experience as a Nurse Practitioner/Provider is preferred, but not necessary,although demonstrated critical thinking skills are required.
- Possess knowledge of Nursing and clinical Nurse Practitioner critical thinking skills application to the aged, chronically ill,and convalescent resident.
- Strong leadership, interpersonal, and problem-solving skills preferred.
- Excellent written and oral communication skills
- Possess traits of punctuality, organizational skills, and ability to function within a team.
- DEA License
- Graduate of an accredited Nurse Practitioner program
